Physician CV
Physician CV template for academic medicine
A physician’s academic CV is the academic CV with clinical sections added: residencies and fellowships, hospital appointments, licensure and board certification, practice activities and clinical innovations. Medical schools ask for it at appointment and promotion. Your school sets the headings, so start from its format and fill in the clinical parts below.

Pick your format. In Format and promotion target, pick your school’s format, or AAMC, a generic academic medical CV, if your school is not listed.
Add your training and hospital roles. Residency and fellowship go in Postdoctoral training, and each hospital role in Hospital and affiliated appointments. Each section prints under your format’s own heading.
Add each license and board. Use Licensure and board certification. The AAMC format prints one line for each: its dates, the credential and the state or board. The Harvard Medical School format prints a table of the year and the type of license or certification.
Log teaching hours and CME. Teaching hours add up by academic year and learners, and CME credits by year and type, for your own reports. School CV formats leave these records out of your CV.
Download Word or PDF. The live preview beside your form shows what the Word file prints. In the Export center, download your format’s Word draft, or choose Print or save as PDF.
